Using Electronic Health Records Data for your Community Health Needs Assessment

Community health is an important priority of hospitals and health systems. Assembling community health data for CHNAs, implementation strategies, and program evaluation is both rewarding and challenging. How do we find data that is detailed enough to reveal health disparities, geographic variability and change over time? Participants will hear an example from Wisconsin of how electronic health record data played an important role in a local community coalition; and will learn the step-by-step process of how to implement this within their own organization.
