The Patient's Voice: Integrating the Community Voice in Care Delivery

As part of the AHA's Hospital Community Cooperative (HCC), member teams are advancing health equity and providing quality care by integrating the collective community voice in the implementation of place-based initiatives. Leaders from HCC teams Holy Name Medical Center and MultiCare Health System will share their hospitals respective journeys towards providing the best screening and educational programs by integrating the voice of the communities they serve.
